How Valley Spring Protects Patient Information

Written Consent Required

Written, signed, time-limited consent is required before any release of identifying clinical information, including to referring providers, payers, and family members. Each release-of-information form specifies what is shared, with whom, for what purpose, and for how long.

HIPAA

Valley Spring follows the HIPAA Privacy Rule and Security Rule for all protected health information. Documentation, transmission, and storage standards are audited annually as part of the CARF accreditation cycle.

Release of Information (ROI)

Written, signed ROI is obtained at admission to authorize communication with referring providers. ROIs specify what may be shared (engagement status, level of care, medication, discharge plan) and the duration of authorization.

Audit Trail

Every disclosure of clinical information is logged in the EHR audit trail, including the requestor, the recipient, the content, and the authorizing ROI. Audit logs are reviewed during CARF and state licensing surveys.
